Completing a consultation form as an agent or representative of an adult patient without the consent of that patient, violates our safety and privacy policy. The patient must be present to sign the form or given consent to sign on their behalf.

You may specifically request that NO information be used whatsoever for promotional purposes, but you must identify any requested restrictions in writing.
